3.0 Completing Department ane of Form I-nine

  • Employees must enter their maiden name or any other legal terminal name they may take used in the Other Last Names Used field. Enter "Due north/A" if the employee has non used other last names.

Employees should enter their home address, flat number, city or town, land and ZIP code. Employees who have no flat number should enter "Due north/A" in that field. Employees who do not have a street accost should enter a description of the location of their residence, such as "2 miles south of I-81, near the water tower."
